Conditions for participation
Participation price includes: Reference Material; access to the website Client Area, support throughout the Proficiency Testing scheme; Exercises guideline and evaluation report corresponding to the chosen exercise(s) (both downloadable from Client Area).
All communications and information related to Qualitycheck Proficiency Testing scheme will be provided by Qualitycheck exclusively and only to the contacts shown in the Table “Data for the management of Proficiency testing scheme”.
According to rule ISO/IEC 17043:2010, Qualitycheck guarantees the anonymity in the presentation of results of the to Qualitycheck Proficiency Testing scheme. Submitted results will be identified in the corresponding reports by a numerical code assigned to the individual laboratories by the organizers.
The Qualitycheck Proficiency Testing scheme will be organized if, and only if, a number of 13 laboratories will subscribe. In case of less than 13 subscriptions, Qualitycheck retains the right to call the exercise off because it is not possible to perform the statistical treatment of the data.
The organizers will communicate any change of time schedule to registered laboratories within 10 working days after registration date expires.
The reference materials will be available for clients for any complaints and appeals up to thirty days from the date of publication of the Final Reports, except for those that, given the particular nature of material, is not available further material at the end of proficiency testing (es.: matrices of wastewater).
Any kind of problem and/or observation regarding the Final Report of Qualitycheck Proficiency Testing scheme, in which the laboratory participates, must be sent to the Qualitycheck no later than 30 (working) days from its publication date.
Any Complaints or Appeals (ref. p. 5.8 ISO/IEC 17043:2010) must be sent to Qualitycheck by mail:
firstname.lastname@example.org. Support service will contact you within three days.
Condition of test samples delivery
Sample delivery: Test samples are delivered by courier service to the address identified in the Table “Data for the management of Proficiency testing scheme” according to the defined time schedule.
Sample substitution: Damaged material will be substituted by courier service at the expense of Qualitycheck.
Shipping costs and conditions: Shipping cost is not included. The shipping cost is a flat rate taking into account the geographic zone + 40 Euro/round in case of order of microbiology proficiency testing (transport in wet ice).
Custom costs: Custom costs are not included and are charged to costumers. Any legal entity or person outside Europe is subject to payment of customs charges.
Charge of Value Added Tax: According to Directive 2008/8/CE VAT has to be charged for the clients of European Community that are not registered for VAT. According to Regulation UE 28/2011 VAT has to be charged for the clients outside of the European Community that are not registered for VAT or other individual similar identification number attributed to the customer by the country of establishment and used to identify businesses or any other proof which demonstrates that the customer is a taxable person.
The billing is round by round. The proforma invoice(s) will be sent you by email after acceptance of the order.
Make your payment directly into our bank account using the ORDER NUMBER as the payment reference.
The invoice will be issued and sent you by e-mail after receiving payment.
The samples will not be shipped until the funds have cleared in our account.
The client may terminate the contract according to art. 1671 of Italian Civil Code. In the particular case of Proficiency Testing schemes:
• Withdrawal is free in case of cancellation within 3 working days after the registration deadline of the Qualitycheck Proficiency Testing scheme in which the laboratory is registered to;
• Customer will have to pay 50% of the cost of each Qualitycheck Proficiency Testing scheme in which it is registered in case of cancellation up to 3 working days before the delivery date of test samples.
Regulatory Law and Jurisdiction
For any dispute, the Italian State Legislation will be applied and the Court of Rome shall have exclusive jurisdiction over any disputes relating to the terms and conditions mentioned above.
Personal Data Treatment
